Recently, another AlloDx study on donor-derived cell-free DNA, "Donor-derived cell-free DNA-based liquid biopsies to determine future kidney transplant rejection," was officially published in the journal Kidney Research and Clinical Practice (DOI: https://doi.org/10.23876/j.krcp.23.286).

This study suggests the potential of donor-derived cell-free DNA (dd-cfDNA)-based liquid kidney biopsy to detect kidney transplant rejection. Multiple previous studies have found that dd-cfDNA can be used to determine the risk of kidney transplant rejection and may be related to antibody-mediated rejection (ABMR), T cell-mediated rejection (TCMR) and estimated glomerular filtration rate (eGFR). High concentrations of dd-cfDNA in body fluids may indicate kidney transplant rejection because dd-cfDNA is released during apoptosis and necrosis triggered by the transplant recipient's immune system. dd-cfDNA tests have advantages over traditional biopsies in that they are non-invasive and therefore have the potential to serve as safe and reliable biomarkers. This study analyzed existing literature and found that dd-cfDNA levels under different injury states have different thresholds, among which ABMR is 2.45%, TCMR is 1.3%, while healthy recipients are 0.44%, and eGFR is 2.5%, which is a 25% decrease compared with healthy recipients. These results suggest that dd-cfDNA levels can be used to signal possible renal rejection. The dd-cfDNA assay is a rapid technique, making it particularly useful in emergency situations, and further research into its use in renal rejection would be beneficial. In addition, the study believes that future use of methylation together with cfDNA monitoring could facilitate the identification of cfDNA sources and further research on transplantation-related issues.
