Published by Clinical Transplantation | The value of ddcfDNA in evaluating acute rejection treatment

Recently, important research results jointly conducted by Professor Wang Rending’s team at the Kidney Disease Center of the First Affiliated Hospital of Zhejiang University and AlloDx Medical Department were published online in Clinical Transplantation, an authoritative international magazine in the field of organ transplantation. This article reveals the use of donor-derived cell-free DNA (ddcfDNA) in the treatment of acute rejection.

Recently, important research results jointly conducted by Professor Wang Rending’s team at the Kidney Disease Center of the First Affiliated Hospital of Zhejiang University and AlloDx Medical Department were published online in Clinical Transplantation, an authoritative international magazine in the field of organ transplantation. This article reveals the evaluation role of donor-derived cell-free DNA (ddcfDNA) in the treatment of acute rejection.

This study enrolled 28 patients with acute rejection, including 5 cases of ABMR, 12 cases of type IA or IB TCMR, and 11 cases of type IIA or IIB TCMR. The changes in blood ddcfDNA content before and after rejection treatment were detected. The results showed that the ddcfDNA content in the patient's blood dropped from 2.566±0.549% to 0.773±0.116% (P < 0.001). There was a positive correlation between changes in Δdd‐cfDNA% and estimated glomerular filtration rate at 1, 3, and 6 months after treatment (crp = 2.570, P = 0.022). This shows that ddcfDNA can be used to detect rejection and evaluate the therapeutic effect. In addition, the use of methylprednisolone affects the content of ddcfDNA.

As a new biomarker for non-invasive diagnosis of organ transplant rejection, ddcfDNA has been widely used in clinical practice abroad, but its role in dynamically evaluating the effectiveness of rejection treatment has not been reported. This study is the first time in the world to systematically clarify the value of ddcfDNA in the treatment of rejection, and further consolidates the theoretical basis for the application of ddcfDNA in the field of organ transplantation.
